Deze gedachten zal ik verder toelichten, met verwijzingen naar de desbetreffende artikelen/onderzoeken. Daarbij zijn onderzoeken die in gerenommeerde bladen zijn gepubliceerd.
Om te beginnen zou het bloedcholesterolgehalte geen directe riscofactor zijn voor hart- en vaatziekten, maar juist functioneren als bescherm/reparatiemateriaal(27, 34, 49).
In onderzoeken vindt men vaak een correlatie wat betreft bloedcholesterolgehalte en hart- en vaatziekten, maar dit wil dan nog niet zeggen dat er een causaal verband bestaat. Een hoog bloedcholesterolgehalte zou ook kunnen ontstaan door hart- en vaatziekten (oorzaak: inactiviteit, roken, overgewicht, DM, stress, micro-organismen etc.) in plaats van andersom (45).According to the modified response to injury hypothesis of atherogenesis, there are at least two pathways leading to the inflammatory and proliferative lesions of the arterial intima. The first involves monocyte and platelet interaction induced by hypercholesterolaemia. The second pathway involves direct stimulation of the endothelium by a number of factors, including smoking, the metabolic consequences of diabetes, hyperhomocysteinemia, iron overload, copper deficiency, oxidized cholesterol, and micro-organisms. There is much evidence to support roles for these factors, but the degree to which each of them participates remains uncertain. However, the lack of exposure-response in the trials between changes in LDL-cholesterol and clinical and angiographic outcome, the inverse association between change of cholesterol and angiographic changes seen in the observational studies, the significant increase in complicated atherosclerotic lesions in the treatment group after cholesterol lowering by diet, and most of all, the fact that high cholesterol predicts longevity rather than mortality in old people, suggests that the role, if any, of high cholesterol must be trivial. The most likely explanation for these findings is that rather than promoting atherosclerosis, high cholesterol may be protective, possibly through its beneficial influence on the immune system (27).
Why is high cholesterol a risk factor in young and middle-aged men? A likely explanation is that men of that age are often in the midst of their professional career. High cholesterol may therefore reflect mental stress, a well-known cause of high cholesterol and also a risk factor for heart disease. Again, high cholesterol is not necessarily the direct cause but may only be a marker. High cholesterol in young and middle-aged men could, for instance, reflect the bodys need for more cholesterol because cholesterol is the building material of many stress hormones. Any possible protective effect of high cholesterol may therefore be counteracted by the negative influence of a stressful life on the vascular system (34).

Het lichaam gebruikt cholesterol als een bouwstof voor het genezingsproces. Dit is de reden dat bij mensen met beschadigde bloedvaten (zoals bij atherosclerose) of longen (tuberculose) behalve calcium, collageen en een aantal andere stoffen, cholesterol wordt aangetroffen in het littekenweefsel dat het lichaam aanmaakt om wonden te genezen (49).
Roken verhoogd bloedcholesterol, maar ook los daarvan de kans op hart- en vaatziekten.
Afvallen verlaagd het bloedcholesterol, maar heeft ook andere gezondheidsvoordelen zoals verbeterde insuline gevoeligheid, minder kans op diabetes of betere instelling.
Overgewicht verhoogd bloedcholesterol, maar zorgt ook voor afname van insulinegevoeligheid en daardoor een grotere kans op diabetes. Dit is een bewezen risicogroep voor het optreden van hart- en vaatziekten.
Beweging verbeterd de ratio LDL/HDL, maar heeft ook vele andere gezondheidsvoordelen zoals sterker hart.
Stress heeft een effect op de ademhaling, de hartslag en de bloeddruk. Het hart helpt daarbij door sneller te kloppen en de druk in de bloedsomloop te verhogen. Zo kan stress bloeddrukverhogend werken. Langdurige stress kan leiden tot chronische klachten en ziekten, zoals maagklachten, overspannenheid en hartproblemen. De weerstand vermindert, waardoor vatbaarheid voor stoornissen en (infectie)ziekten toeneemt (51). Daarnaast veranderd de ratio LDL/HDL.
Dit zou verklaren waarom het cholesterolgehalte een voorspellende factor kan zijn voor het optreden van hart- en vaatziekten en er een correlatie tussen deze twee bestaat.
Er zijn diverse onderzoeken die laten zien dat verzadigde vetten het bloedcholesterolgehalte laten stijgen. Vaak wordt dan de oorzakelijke link gelegd met hart- en vaatziekten wat in dat onderzoek vaak niet is aangetoond. Men gaat dan uit van intermediate endpoints zoals J.V. Woodsite en Daan Kromhout aangeven (2005):
Met name Uffe Ravnskov (The Cholesterol Myths) is naast Mary Enig een bekend persoon die negatieve kritiek uit op de huidige vet/cholesterolopvattingen op basis van zijn onderzoeken (27-33,34). Hij staat echter niet alleen in zijn opvattingen gezien de leden van THINCS.Most of the knowledge about the effects of dietary fatty acids on CHD risk is based on observational studies and controlled dietary experiments with intermediate end points (e.g. blood lipoprotein fractions). Information from high-quality randomised controlled trials on fatty acids and CHD is lacking (36).
Van 26 lange termijn(4-23 jaar) follow-up studies (1-26) die de relatie onderzochten naar verzadigde vetten en het optreden van hart- en vaatziekten, hebben er schijnbaar slechts vier een statistisch positief verband gevonden.
Een ander onderzoek bestaande uit 22 interventiestudies trekt de volgende conclusie:
Nog een ander onderzoek laat zien dat een hoger percentage verzadigd vetgebruik, negatief is gecorreleerd aan het ontwikkelen van atherosclerose 37. Ook zijn er onderzoeken gedaan waaruit blijkt dat mensen met de hoogste cholesterolwaardes, de laagste mortaliteit hebben en het langste leven (34, 47) , of dat er helemaal geen verband bestaat tussen bloedcholesterolwaardes en het optreden van hart- en vaatziekten.Lowering serum cholesterol concentrations does not reduce mortality and is unlikely to prevent coronary heart disease. Claims of the opposite are based on preferential citation of supportive trials (33) .
In landen van de Seven Countries studie van Ancel Keys worden al grote verschillen gevonden (30 )
Deze vragen heb ik ook voorgelegd aan het Voedingscentrum, de Nederlandse Hartstichting en een hoogleraar humane voeding.Eigenlijk is zelfs nog nooit onomstotelijk bewezen dat cholesterol de hoofdschuldige is. Eén van die opmerkelijke constateringen is dat veel mensen die lijden aan hart- en vaatziekten helemaal geen verhoogd cholesterol gehalte in het bloed hebben. Ook vanuit andere gezichtspunten kunnen vraagtekens gezet worden bij de algemene geldigheid van de theorie dat cholesterol de hoofdschuldige is in het hart- en vaatziekte verhaal. Veel volkeren over de hele wereld krijgen via het eten veel meer cholesterol binnen dan de gemiddelde West Europeaan, terwijl het aantal hart- en vaatziekten bij die volkeren verwaarloosbaar klein is!( 48 )
Controversy exists about the roles that dietary fat and cholesterol play in the risk of heart disease. Confounding factors influence the interpretation of results of epidemiologic studies. For example, over the years, France and Finlandpopulations that have similar intakes of cholesterol and saturated fathave consistently had very different mortality rates from CAD. Some epidemiologic evidence suggests that consumption of high amounts of saturated fat and cholesterol lead to high blood cholesterol and thus to an increased risk of heart disease (for review see reference. Reviews of studies that linked dietary cholesterol and fats and high serum cholesterol with atherosclerosis and cardiovascular disease aptly pointed out that the results of the epidemiologic and experimental studies are inconclusive or even contradictory. The conclusion of an analysis of the history and politics behind the diet-heart hypothesis was that after 50 y of research, there was no evidence that a diet low in saturated fat prolongs life. The proposition that dietary fat is unhealthy is based on the fact that high intakes of saturated fat elevate blood cholesterol and thus increase the incidence of atherosclerosis, which then increases the risk of CAD (50 ).
Het antwoord van de Nederlandse Hartstichting was in eerste instantie summier. Er werd verwezen naar een onderzoek van Walter Willet (mediterrane dieet) en een onderzoek van Martijn Katan en Ronald Mensink (effect van vetzuren op lipoproteïnen) maar deze onderzoeken beantwoorden niet mijn vragen. Het mediterrane dieet is heel breed en sluit andere factoren niet uit.
Effecten van vetzuren op lipiden en lipoproteïnen geven geen causaal verband met hart- en vaatziekten, maar intermediate endpoinst.
Een tweede keer de Nederlandse Hartstichting geraadpleegd te hebben (afdeling onderzoek) kreeg ik een onderzoek toegestuurd, een meta-analyse van Stewart Truswell.
Dit onderzoek lijkt inderdaad een pleidooi voor de huidige cholesterolopvatting. De voedingsinterventies bestonden echter voornamelijk uit verlaging van verzadigd vet en een toename van onverzadigd vet en soms vis. Alfa-linoleenzuur en vis zijn los van cholesterolwaardes al negatief gecorreleerd aan hart- en vaatziekten in dat onderzoek. Bovendien is in sommige onderzoeken gewichtsvermindering en verandering in leefstijl geadviseerd. Het onderzoek beschrijft dieetmaatregelen en het effect daarvan op hartaandoeningen en totale sterfte. Dit maakt volgens mij de uitkomst van bloedcholesterolconcentratie als causale factor discutabel, ondanks de aanwezige correlatie.
De hoogleraar gaf een verwijzing naar de onderzoeken van D. Steinberg (40-44).
Hoewel dit een pleidooi is voor de huidige verzadigdvet/cholesterolopvattingen kan dit niet de diverse resultaten verklaren van onderzoeken met een andere uitkomst.
Het Voedingscentrum vond het niet tot hun taak behoren om dergelijke mails van studenten te beantwoorden.
Op de opleiding wordt je verteld dat het cholesterolgehalte een risicofactor is voor hart- en vaatziekten.
Het is echter nooit verkeerd om sceptisch te zijn en niet blind te varen op een standpunt van iemand.
Nadat ik mezelf er verder in verdiept heb kom ik dusdanig veel onderzoeken tegen die de huidige cholesterolopvattingen weerleggen waardoor het voor mij niet meer zo duidelijk is.
Van overtuigend bewijs kan ik niet spreken.
Samenvattend zou ik jullie graag de volgende vragen willen voorleggen:
In welke hiërarchie moet ik het totaal aan onderzoeken plaatsen om de juiste conclusie te kunnen trekken en te kunnen onderbouwen? Er zijn namelijk redelijk wat onderzoeken tegen de huidige cholesterolopvattingen die ik (nog) niet zomaar naast me neer wil/kan leggen. Zowel op interventie-, als epidemiologisch niveau is er onduidelijkheid.
Wat kan gebruikt worden als tegenargument -onderzoek op de onderstaande onderzoeken (o.a. 27-39), met een zo mogelijk hogere bewijslast? Dit moet mogelijk zijn gezien het huidige algemene standpunt over verzadigd vet en cholesterol als risicofactor voor hart- en vaatziekten.
En een andere, maar eveneens belangrijke vraag voor mij, hoe moet ik de onderstaande onderzoeken interpreteren en uitleggen? Alles bij elkaar heb ik redelijk wat onderzoeken gevonden die de huidige vet/cholesterolopvattingen niet ondersteunen. Als deze onderzoeken correct zijn zou de huidige vet/cholesterolopvatting namelijk herzien moeten worden.
Iedereen is er van overtuigd dat cholesterol een primaire risicofactor is voor hart- en vaatziekten. Uit onderzoek blijkt deze vlekkeloze overtuiging niet.
In deze post heb ik geprobeerd om mijn probleem/vragen duidelijk te maken.
Naar ik hoop is dat gelukt en kan iemand van jullie een antwoord/reactie geven.
Probleem is dus dat uit diverse onderzoeken blijkt dat het cholesterolgehalte geen directe risicofactor is voor het optreden van hart- en vaatziekten. Daarnaast zijn er ook onderzoeken die weer het tegengestelde concluderen
Ik ben op de hoogte van de verschillende soorten wetenschappelijk onderzoek en de bewijslast die daaraan verbonden is. Maar zowel op epidemiologisch- als op interventieniveau worden tegenstrijdige conclusies getrokken.
Welke conclusie kan nu wetenschappelijk gezien het beste getrokken worden, met de hoogste bewijslast?
