Op zich zou dit eigenlijk niet zo heel erg verrassend mogen zijn, maar toch denken veel te weinig onderzoekers nog in die richting... En ik vermoed dat ons tijdperk, waarin de nadruk vooral wordt gelegd op de biologische psychiatrie/ psychopathologie, daar voor iets tussen zit...

Uit deze studie bleek dat mensen die een depressie doormaakten én daarbij in een belastende sociale situatie verwikkeld zaten, consequent minder goed reageerden op de behandeling, onafhankelijk van wat die behandeling precies inhield (SSRI's en/ of ondersteuning). Het ging dan vooral om sociale situaties waarbij de persoon een gevoel ervaarde van vernedering of van in de val te zitten.
Background

The role of current social risk factors in moderating the impact of antidepressant medication has not previously been explored.

Method

In a RCT of SSRIs of general practice patients with mild to moderate depression (HDRS 12–19) two social indices of aversive experience were developed on the basis of prior research. First, the Life Events and Difficulties Schedule (LEDS) was used twice to document: i) recent stressful experience prior to baseline, and ii) after baseline and before follow up at 12 weeks both stressful and positive experiences, taking account of ‘fresh start’ and ‘difficulty-reduction’ events. Second, an index of unemployment-entrapment at baseline was developed for the current project. The HDRS was used to measure outcome as a continuous score and as a cut-point representing improvement below score 8.

Results

Each social index (LEDS and Unemployment-entrapment) was associated with a lower chance of remission at 12 weeks and each was required to model remission along with treatment arm. However there was no interaction: the degree of increased remission for those randomised to SSRIs plus supportive care compared to that for those with supportive care alone was the same regardless of social context.

Limitations

Dating of remission was not as thorough as in previous work with the LEDS. Detailed examination of positive experiences suggested the large majority were not the result of remitting symptoms, but it is impossible to rule this out altogether.

Conclusions

Remission rates among patients in aversive social contexts are consistently much lower irrespective of treatment. There is thus a need to evaluate the efficacy of alternative more socially focussed interventions for depressive conditions likely to take a chronic course in general practice

Voor mij is dit een aanwijzing dat psychotherapie wel degelijk iets kan bieden bij depressieve mensen. Met daarbij de nadruk op hoe de therapie zo'n cliënt kan helpen zodat hij zélf stappen gaat zetten om uit de impasse te geraken.

Al is dit vermoedelijk een doel waar de meeste therapeuten naar streven (maar wat natuurlijk niet altijd lukt)


Een andere bedenking is dat ik me herinner dat het vooral de eerste depressie is, die gelinkt is met het doormaken van stressvolle life events. Als iemand recidiveert (en dat gebeurt erg vaak), dan blijkt de samenhang met stressvolle life events steeds zwakker te worden. Met andere woorden: er is steeds minder nodig opdat de persoon terug depressief zal worden.

Ik vind de samenvatting nogal onduidelijk. Klopt het dat men twee zaken trachtte te meten. De effectiviteit van antidepressiva als aanvulling op psychotherapie (iedereen kreeg psychotherapie) met als conclusie dat er geen effect was en 2)Hoe de mate van recente psychologische stress gerelateerd is aan de kans op spoedig herstel met als conclusie dat hoe groter de psychologische stress waaronder iemand gebukt ging hoe geringer de kans op herstel. :eusa_whistle:

Adpruys, ik heb even wat gezocht op Google om iets meer info over de studie te verzamelen. En ik stootte daarbij op de volgende beschrijving, die sowieso een stuk duidelijker is, denk ik:
In their study, 220 patients with significant depression symptoms were randomly assigned to either supportive care or to SSRIs plus supportive care (SSRIs are the class of medications most commonly used to treat depression such as Prozac and Paxil).


The investigators undertook a detailed assessment of each person's social environment (both events that were negative and positive in nature) at the beginning of the study and 12 weeks later, after the treatments had begun.


What they found was that those patients who faced significant environmental adversity at any point were only half as likely to respond to the treatments and remit from depression than patients who were in more benign environments. It did not matter what kind of treatment the patients received. Remission rates among patients in aversive social contexts were much lower irrespective of treatment type. In fact, only 1/5th (!) of those in aversive social contexts remitted after 12 weeks of treatment.

Verder is het belangrijk dat de studie huisartspraktijken bekeek. En dat betekent, volgens mij, dat 'supportive care' niet hetzelfde is als psychotherapie. Maar dat het hier gaat om een vorm van ondersteuning (dus vermoedelijk steunende gesprekken door de huisarts met de patiënt).
